(b) Organizational Description

All EOMHC facilities and programs have a written organizational description which is approved by its governing authority (See Appendix). EOMHC makes the organizational description available to staff and, upon request, to the public upon request.

EOMHC Facility

We offer a publicly accessible facility that has office space, individual and group counseling space, secure records storage, and provides protection of consumer confidentiality.

EOMHC-LLC will assure there is adequate space for each program to accommodate the need for privacy during visits and therapeutic interventions.

Our goal is to provide quality, community-based, trauma based therapies and rehabilitative services for Mental Health and Co-Occurring disorder to individuals in need through counseling, education, and advocacy.

EOMHC-LLC is dedicated to:

  1. Providing individualized, quality services in the least restrictive settings.

  2. Offering a continuum of services with an interdisciplinary approach.

  3. Aiding all individuals regardless of race, color, creed, or belief.

  4. Serving individuals regardless of race, color, disabilities, creed, or belief.

  5. Preserving the family as the foundation of society.

EOMHC-LLC believes it is the right of every individual, regardless of race, gender, age, disability, or socioeconomic status to have the highest quality of services available. EOMHC-LLC believes this philosophy to be the cornerstone for planning, development, and coordination of a quality service delivery system. We believe that through a dedicated staff we can provide these services.

B1: Population to be served

EOMHC-LLC was created to promote increased Services, as deemed appropriate, to rural Oklahomans of any race or gender or disability, children and adolescents (6-17), as well as, adults (18-90+). EOMHC-LLC is able to provide Mental Health & co-occurring Substance Abuse treatment. Many of our emergency populations are presenting with co-occurring issues that also reflect trauma based therapies for those that are identified as ‘in need’ for the same services & substance abuse.

B2: Mission Statement

EOMHC-LLC’s mission is to meet the diverse needs of its clients by using approaches that are designed to be flexible, client-centered, family-focused, culturally responsive and empirically-based. We empower families and individuals by promoting collaboration between our clients, their community supports (family, friends) and our behavioral associates to improve their behavioral and mental health outcomes.

B3: Annual Goals and Objectives

Goals:

A Provide quality adult mental health/behavioral health services including individual/family/group services
B Provide quality children’s/adolescent mental health/behavioral health services including individual.
C Provide quality case management
D Provide Recovery Oriented services including intensive outpatient/inpatient referrals/follow-up services.

  1. Case Management

  2. Referral to Inpatient / Outpatient recovery programs both local/state and federal.

  3. Educate consumers as to various recovery systems/approaches through individual counseling and available literature.

E Provide Culturally Competent services for identified local individuals and groups.

  1. Provide cultural training through local indigenous groups.

  2. Invite local/cultural groups to participate in services.

  3. Provide literature review of culturally competent material from, ACA, APA, Internet Resources, and other appropriate entities.

  4. Targeting language and access barriers best practices.

  5. Identifying weaknesses and access issues.

F Provide Trauma informed services/crisis management/resource acquisition.

  1. Provide access/information on trauma (treatment/local and state resources)

  2. Crisis Management utilizing 24 hr phone service hotline.

  3. Educating consumers through orientation materials / counseling on crisis resolution. / Resource availability.

    Objectives:

  4. Conduct yearly training of service providers for credentialing on cultural competencies / indigenous group competencies / trauma informed services / crisis intervention services

  5. Provide literature reviews of services provided

  6. Quality control of chart/charting procedures

  7. Actively involve consumers in choices regarding services

  8. Problem solving/review of critical incidents

  9. Consumer satisfaction/feedback survey

EOMHC provides services in the following counties within Oklahoma:

  1. Leflore

  2. Wagoner

  3. Latimer

  4. Muskogee

  5. Eastern Haskell

  6. Northern Pushmataha

  7. Northern McCurtain

  8. Sequoyah

  9. Southern Cherokee

EOMHC-LLC shall make organization description, mission statement, and annual goals available to all staff and the general public upon request through Orientations of staff and Consumer Orientations.
