450-1-9-5.4 Core organizational standards
(a) Governing Authority.
All facilities and programs of EOMHC have documents of authority, which is available to the public and ODMHSAS upon request. Documents of authority identify the duly constituted authority and governance structure for assuring legal responsibility and for requiring accountability for performance and operation of EOMHC (including all components and satellites).
(1) Minimum eligibility criteria for governing body membership:
Masters Degree in the Behavioral Health Field
Full Licensure in the State of Oklahoma for LPC, LCSW, LMFT, LADC
Minimum of 3 years of Experience.
(2) There are 2 membership positions:
Owner, CEO: Edward Williams
Owner, Clinical Supervisor: Sue Williams
(3) Members are selected by:
qualifications of licensure,
clinical ethics,
work ethics,
experience.
Length of employment at EOMHC
(4) The number of members necessary for a quarum is 1 (one) providing that a witness within EOMHC is present at time of Governing Operations.
(5) EOMHC shall have quarterly governing meetings that include the CEO, Clinical Director, All officers, Assistants.
(6) All appointments are in perpetuity for governing body membership and is under the sole descretion of the CEO. If CEO is unable to perform duties necessary to facilitate operational standards for appointments within the governing body, the Clinical Director shall provide operational duties of the CEO.
(7) The CEO shall have final jurisdiction on all matters of operation with the exception of clinical operations. The Clinical Director shall have authority in all matters of clinical operations.
